Aetna and Banner Aetna:

We are preferred providers for Aetna, and can bill directly for lactation consultations for most Aetna and Banner Aetna subscribers. Please check your individual plan to make sure.

Cigna PPO, Blue Cross Blue Shield PPO, Humana, and United Healthcare PPO:

We use The Lactation Network to bill your insurance. Most families are eligible for at least 6 visits. To qualify, please complete their intake form. Choose: “ I already have an IBCLC that I’d like to be paired with” and enter Arizona Breastfeeding Center. We should hear back on coverage by the next business day.

Other Insurance Providers:

We accept HSA and FSA cards, and will provide you with a superbill to submit to your insurance for reimbursement.

AHCCCS Plans:

AHCCCS does not currently cover lactation support services, however we do have low cost visits available for AHCCCS members. Please choose “AHCCCS/WIC Low Cost Visit”
